Medication Review
If you are unable to order from your repeat prescription list, you may be due a medication review.
Medication reviews help ensure your prescriptions remain safe, effective, and appropriate for your needs. They may also include a review of any long-term health conditions.

Medication reviews are often linked to your long-term condition review and may include:
- Weight and Body Mass Index (BMI)
- Blood pressure and pulse checks
- Blood tests
You will usually be invited for a review once a year, or more frequently depending on your individual needs. Invitations are sent via text message or letter.
These checks are designed to identify any potential issues early and support your ongoing health.
Long-Term Condition Reviews
We provide regular reviews for patients with the following conditions:
- Hypertension (high blood pressure) and high cholesterol (including those on statins)
- Diabetes
- Ischaemic heart disease, heart failure, chronic kidney disease, and stroke
- Asthma and Chronic Obstructive Pulmonary Disease (COPD)
- Learning disabilities
- Thyroid conditions (hypothyroidism and hyperthyroidism)
- Rheumatoid arthritis and patients on immunosuppressant medication (if not under hospital care)
- Severe mental illness and dementia
Structured Medication Review
A structured medication review may be offered if you:
Have a complex long-term condition and take 10 or more regular medications
- Are living with severe frailty, are at risk of falls, are housebound, or live in a care home
- Take addictive pain medications
- Are prescribed high-risk medications
This is a comprehensive, person-centred consultation with a clinician. The aim is to optimise your medication by:
- Reviewing how well your medicines are working
- Reducing or stopping unnecessary medications
- Adjusting doses to minimise side effects
- Helping you better understand your treatment
