Self-Referrals

Some referrals to specialists/specialist services require you to see a clinician. However, there are some services for which you are able to refer yourself which are listed below.

Please carefully consider the following:

  • You have reviewed the inclusion/exclusion criteria for each service.
  • Self-referrals are usually triaged by the service who will assess your suitability before accepting your referral and providing appointments.
  • If you’re referral is assessed and deemed to be unsuitable, you may be re-directed back to your GP surgery.
  • Please ensure you complete self-referral forms, adding as much detail as possible.
