Sick Note / Fit for Work Certificate

If you have been off work for less than 7 days, you do not need a fit note. Instead, you should complete a self-certification form (SC2), which is available from your employer or on the HMRC website.

If you have been unwell and off work for more than 7 days, you may need to see a clinician. However, if you have been under the care of the hospital your GP may be able to issue the fit note based on the hospital documentation. You can request a medical certificate (fit note) by completing an eConsult form.

If you need to extend an existing fit note, please complete another eConsult form.

Please note:

  • Fit notes can be backdated where appropriate
  • There is no need to book an urgent or emergency appointment for a fit note request
  • If your employer has requested a private sick note for illness less than 7 days, this can be issued and a fee is payable.
